Should I get myself diagnosed?

yes, you need to seek help with a psychiatrist for a diagnosis and possible treatment. also going to see a talk therapist wouldn't hurt either. i see both and they are working together to help me with many of the problems that you mentioned plus some that you didn't. i am doing much better since going to a talk therapist. it is amazing what you can find out about yourself that you never even expected. such as i didn't realize how much anger i had inside myself that had a lot of control over what i do to myself and others. now that i know that a lot of what i do to myself has stopped. these things don't just start over night and they don't go away over night but they do get better with time and hard work on your part.
